Work directly with the management team to establish and deliver Clinical Care Coordination services for Rockhampton.

As a Clinical Care Coordinator, you’re a pivotal member of a multidisciplinary team contributing high quality evidence based clinical care, focusing on early intervention and support of people who require psychological and psychosocial services.

  • You will deliver evidence-based psychological interventions, manage a clinical caseload, and conduct risk assessments with detailed management plans.
  • You’ll provide client-centred care, integrating family support where appropriate.
  • You’ll coordinate care plans, mobilise complementary services, and ensure seamless transitions to tertiary care when needed.
  • Upholding confidentiality and ethical standards, you’ll participate in quality improvement activities and contribute to positive client outcomes.

For this interesting and rewarding role, you will need to be a committed nursing or allied health graduate.  While accredited Mental Health Clinicians (such as Psychologists, Social Workers or Mental Health Nurses) are preferred due to their expertise in providing therapeutic support, new graduates are also welcome.  AHPRA registration or membership with AASW is essential.
